For those clients who wish to book the services of East Anglia EMS, we have provided several online forms for you to download, complete & return for a more streamlined booking process.

The documents are arranged by title, please ensure that you download the correct form for your required service. PDF Downloads:

Event Medical Request Form: For all medical event requests. This is a two page document that requires all sections to be completed fully and returned to East Anglia EMS either by Fax or by Post.

Patient Transport Request Form: For all transport requests of patients. This form is to be used for both Urgent and Non-Urgent PTS Requests. Please complete all sections in full and return to East Anglia EMS by Fax or Post.

Air Ambulance Request Form: For all initial Air Ambulance Requests. Please note that in most cases we will be required to complete additional specific paperwork and medical clearance documentation. We would ask that if you have any questions to contact our Repatriation Co-Ordinators first to establish the correct paperwork that will be required.

Standard Terms and Conditions (Event Cover): Please read this document as it contains important information about our event cover terms and forms the agreement of EMS providing event cover to our clients.
